New Client Agreement
Veterinary Release Form
I, ____________________________, authorize Must Love Pawz, LLC (“MLP”) to seek veterinary treatment for my animal(s) in the event of an emergency or if any medical condition arises while in their care.
I understand that every effort will be made to contact me or my designated emergency contact prior to obtaining medical treatment for my animal(s). However, if my emergency contact or I cannot be reached in a reasonable amount of time or if immediate medical attention is required, I give MLP permission to seek treatment for my animal(s) at the veterinary clinic listed below.
I agree to pay all expenses related to any veterinary care deemed necessary by MLP for my animal(s). I understand that MLP will make every effort to contact me or my emergency contact prior to incurring any veterinary expenses, but in the event that I cannot be reached, I authorize MLP to make any decisions regarding my animal(s)’ medical treatment.
I further acknowledge that MLP is not responsible for any pre-existing medical conditions, injuries, or illnesses that my animal(s) may have, and that I am responsible for disclosing any such conditions to MLP prior to the start of their care.
In the event that MLP incurs any expenses related to my animal(s)’ veterinary care, I agree to reimburse MLP for these expenses within 14 days of receiving an itemized bill or receipt. I understand that failure to reimburse MLP in a timely manner may result in additional fees and/or legal action.
I have read and understood the terms of this Veterinary Release Form and agree to all conditions outlined herein.


Service Agreement
1. Any emergencies or urgent needs may be addressed by calling or texting Dan Savage at (518)461-1641 Or (518)313-6433. If you are unable to reach Dan, you may call Must Love Pawz, LLC directly between 8:00 AM and 8:00 PM EST.

2. Please notify us upon your return home. If we haven’t received
confirmation of your arrival, we will continue visiting to ensure the safety and well-being of your pet(s). Any unscheduled services may be charged at double the normal rate, and payment will be due in full within 5 days of your arrival home. Please call or text and leave a message that you have returned.

3. Always confirm your reservations with us via phone or email. We cannot accept a message left as a reservation. This ensures that we won’t miss your messages and your pet(s) won’t be left alone. If you need to extend your trip, we will only accept extensions by direct confirmation.

4. Payment is due in full 7 days before your departure. For repeat clients, you may leave a check for the full amount to be picked up on the first scheduled visit. We also accept all major credit cards.

5. CANCELLATIONS: Except for holiday periods, cancellations may be made up to 24 hours in advance of the first scheduled visit. After that period, a cancellation fee of 25% will be charged. Cancellations during holiday periods may be made up to 7 days in advance of the first scheduled visit. After that period, a cancellation fee of 50% will be charged. Cancellations of less than 48 hours will be due in full.

6.Visits will be made between the hours of 6:00 AM and 9:00 PM EST. Visits for dogs will be no more than twelve hours apart. Cat visits will be at approximately the same time each day. Cats must be visited at least once per day to ensure their safety and wellbeing. Specific time frames for your visit(s) will be discussed prior to each reservation.

7. There will be a $10 delivery fee if the Pet Sitter needs to pick up supplies for your pet(s). Please remember to leave adequate (or more) food, litter, leashes, etc. for your pet’s needs.

8. HOLIDAYS: There will be a $5.00 fee per visit in addition to the regular per visit fee charged for services on the following holidays: New Year’s Day, Easter, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day, Christmas Eve, and Christmas Day.

9. PEAK DATES: There will be a $5.00 fee per day in addition to the regular per visit fee charged for services that occur during our peak dates. The peak date calendar will be emailed out at the start of each year. Physical copies can be provided by request.

10. RETURNED CHECK CHARGE: Client agrees to pay a $25.00 fee for each check returned by the client’s bank, regardless of the reason.

11. KEY PICKUP AND RETURN POLICY: Must Love Pawz LLC will retain a key at the time of the contract signing (when applicable). Your key will be returned within 10 days of the end of the contract period for the first Pet Sitting Contract at your request. It is suggested that the client allow Must Love Pawz LLC to retain a key on file for future pet sitting needs. A charge of $10.00 will thereafter be charged for the key pick up. Must Love Pawz LLC will agree to keep your key locked in a safe place with only secret coded identification to you and your address.

12. UNSECURED PETS: Must Love Pawz LLC will not be held liable for free roaming outdoor pets (i.e. cats) in the event of illness, injury, loss, or death. It is strongly advised that all pets wear an ID tag with a contact number and that they remain inside the home or confined area for their own safety and welfare in your absence.

13.SECURED AREAS: It is the pet owner’s sole responsibility to pet proof any areas of the home and/or property to which the pet(s) have access. This includes thoroughly inspecting fences, gates, latches, doors, and other devices meant to keep the pet(s) inside or away from any areas pet(s) may have access. The Pet Sitter does not assume and has no liability for any injuries the pet(s) may sustain while in its own home/property.

14. NON-WORKING KEYS: Must Love Pawz LLC is not responsible for locksmith fees incurred due to non-working keys.

15. INVISIBLE FENCING: Invisible fencing will be considered off-lead. Owner assumes all liability in case of loss, theft, or damage to pets, people, or property when invisible fences are relied upon as confinement.

16. PET PICTURES: Must Love Pawz LLC requests permission to take pictures of your pets for our use in a future email, newsletter, website, or on social media. Your privacy will be protected at all times. Your names will never be used without permission.

17. BITE CLAUSE: As a client of Must Love Pawz LLC, I (or my homeowner’s insurance) will be responsible for any injury (i.e. pet bite, scratches requiring medical attention) to my Pet Sitter due to my pet(s) or condition of my premises.

18. LIABILITY WAIVER: I, ___________________________________, do hereby waive and release Must Love Pawz LLC and its employees, agent or Independent Contractors from any liability of any nature, for injury, death, or missing pet(s) which may occur from the acts or omissions of any person or persons other than Must Love Pawz LLC and its employees, agents or Independent Contractors. Further, Must Love Pawz LLC and its employees, agents or Independent Contractors, is hereby released from and against any and all liability of any nature in the event that anyone other than Must Love Pawz LLC and its employees, agents or Independent Contractors has access to the premises and pet(s) during the term of contracted jobs.

19. This agreement is perpetual until rescinded by either party; it permits Must Love Pawz LLC to accept all future reservations by phone call, text, email, or form submission via the Pet Sitting Software System.

20.VISITING HOURS: Although we will try to arrive as close to a requested time as possible, we cannot guarantee specific times.

21. PLANTS/GARDENS: Must Love Pawz LLC is not responsible for wilted, dead, or otherwise unhealthy plants. We will attempt to follow your directions as precisely as possible within the time allotted, but cannot be responsible if the results are not favorable. Please place all plants together on a waterproof surface in plain sight, as your Pet Sitter is not responsible for water damaged areas or missing plants. For outdoor gardens/plants, please have sprinklers in place so a pet sitter only has to turn on the water faucet. Please be sure water faucet is working properly; faucets in obvious need of repair will not be utilized by the Pet Sitter. Strategically placed water pots are also requested.

22.Client will keep Must Love Pawz LLC updated on any changes to the initial information provided. Must Love Pawz LLC cannot be held liable when it has not been provided with updated information.
